Check the Question Category Settings under the Detailed Settings to make sure the boxes for the course questions and faculty questions are checked.  The system will default to checking these boxes when the survey is first set-up, but if questions are added or removed, it will uncheck these boxes.  Its best practice to check a few of the Question Category Settings when building the survey for proper previewing.
