This may be related to the Results Access settings under the Survey List. Be sure that evaluation results are available either during or after a survey is open or closed. Next, check the settings for instructors and assessed individuals to make sure they are appropriately set according to who should see what. Check the Question Category Settings to verify that the course and instructor questions are checked.
