Yes. The message will be emailed to the Course Evaluations Manager with the survey name, a list of categories to choose from (if there is more than one), and a pop-up help message asking the student to include the details that will make the assignment change easier to manage.
This option is set up in the Display Options of the survey administration, under Survey Participant Options, "Allow participants to request additional individuals to evaluate."
